This is a time to ask questions and share how you are really feeling, details that can inform a better tailored care plan. It is important to speak up about any additional health conditions or medications. In preparation for Dialysis treatment, a master plan is designed to manage the details and guide you through the various aspects of your prescribed treatment. This plan, known as a Care Plan, will cover dialysis routine, medications, diet and treatment for other health conditions.
This care plan is reviewed regularly to measure its effectiveness.

Dialysis access refers to the connection to treatment and the key to feeling your best and maintaining your health.
Depending on the dialysis treatment type you are receiving. You and your attending doctor may discuss your overall health, lifestyle, your veins and their accessibility, plus any history that you may have had with previous access sites.
This is an access site that is formed by the joining of a vein and an artery in the arm. When it is used, it requires 6-8 weeks to heal before use.
It is the preferred dialysis access for hemodialysis, if one is eligible for this type of access. It is a more natural access as it is formed by one’s own blood vessels.
A fistula offers better blood flow and lowers the chances of infection and clotting, compared to a Graft or a Catheter.
This is an access site that is formed by a piece of soft tubing that connects a vein and an artery in the arm. It requires 2-3 weeks to heal before use.
This access is a better alternative if one’s veins are too small or weak for a fistula.
A graft offers better blood flow and lowers the chances of infection compared to a hemodialysis catheter.
This is an access created by a tube placed in the neck, chest, or groin, and is often used temporarily while a Graft or Fistula heals. It doesn’t have any required healing time, but it is not recommended for long-term use, except the IntraJugular access route which can be used for up to 9months.
A hemodialysis catheter has a higher risk of infection and clotting due to the proximity of the catheter tip to the heart.
This access site must be kept dry, meaning limited bathing options and swimming is not recommended.
